Trillium Medicaid Providers: Take the 2024 Social Needs Screening Inventory Survey

Trillium is launching this Social Needs Screening Inventory Survey to gather responses necessary to pass the new Social Needs Screening and Referral CCO incentive metric. The survey will be used to conduct an environmental scan to help us identify how members are being seen, the capacity of health-related social needs referrals, resources available, and the gaps that must be addressed.
 
Organizations who complete this survey by October 4 may be eligible to earn a bonus which is outlined within Trillium's 2024 CCO Quality Pool Bonus Funds Program. Thank you for your partnership in helping our members stay healthy.

Live Healthy Lane 2024 Community Partner Assessment Survey

Every five years Live Healthy Lane conducts a comprehensive assessment to understand the health of our community and the conditions that support health in Lane County. The first assessment to launch this year is called the Community Partner Assessment. This is an organizational assessment to determine our community’s overall capacity to improve health and address health inequities. 

We’re asking organizations large and small across Lane County to participate so that we can get a full picture of: 

  • The different roles community organizations play in improving health
  • Assess the skills, knowledge, and capacity of our community as a whole
  • Describe the collective strengths and opportunities that exist to improve health and address inequities
  • Identify opportunities to build community power to improve health

The survey will be open until the end of September.

Lane County Public Health Shares Resources from OHA's JH Baxter Health Consultation

Oregon Health Authority (OHA) released the final version of their JH Baxter Health Consultation. You can now read the final report (PDF) as well as summaries in English (PDF) or Spanish (PDF). The report details the health impacts and strategies for reducing harm from contamination linked with the JH Baxter Wood Processing Plant in West Eugene. During residential soil sampling in the neighborhood bordering the wood processing plant, the Oregon Department of Environmental Quality (DEQ) and U.S. Environmental Protection Agency (EPA) identified several yards with elevated levels of dioxins, including seven (7) with levels at or above 40 parts per trillion (ppt).
