Due October 31: Social Needs Screening Inventory Survey for Trillium Medicaid Providers

Thank you to all of our Trillium Medicaid providers who have completed the 2024 Social Needs Screening Inventory Survey!

If you have not yet submitted your responses, please complete this survey by October 31, 2024. Organizations who complete this survey by October 31 may be eligible to earn a bonus which is outlined within Trillium's 2024 CCO Quality Pool Bonus Funds Program.

This Social Needs Screening Inventory Survey gathers the responses necessary to pass the new Social Needs Screening and Referral CCO incentive metric. The survey will be used to conduct an environmental scan to help us identify how members are being seen, the capacity of health-related social needs referrals, resources available, and the gaps that must be addressed.

Mark Your Calendars for Trillium Provider Collaborative: Point Click Care Portal

Please join us for Trillium's Fourth Quarter Provider Collaborative! We'll discuss the Point Click Care (PCC) portal, formally Collective Medical, and how it can be used to address the Initiation and Engagement of SUDs Treatment (IET) measure.

Guest speakers Matthew Ver Eecke and Sarah Watt from Point Click Care will give a demo of the PCC Portal and the IET Cohort offered by Trillium. Please come prepared with questions to ask the experts on this helpful tool!

Trillium Medicaid Providers: Complete the Required Provider Health Equity & Technology Census by  December 31

Trillium is committed to partnering with you to support culturally and linguistically appropriate care for our members.
Every year, the Oregon Health Authority (OHA) requires Trillium to collect data from providers about health equity, language services, disability access and telehealth.

Trillium and OHA use this information to:

  • Identify areas where we need to expand our network
  • Determine electronic health record (EHR) needs
  • Inform community health equity initiatives
  • Develop provider resources to help improve access to care and health outcomes

Please submit your data by 5 p.m. on Tuesday, December 31, 2024 using Trillium’s Provider Health Equity & Technology Census Form. The form takes approximately 30 minutes to complete.
